Liposuction, also known as lipoplasty or suction lipectomy, is the process of removing unwanted fat from specific areas of the body. The most common treatment areas include the upper outer thigh area known as “saddlebags”, the abdomen, the flanks or “love handles”. Liposuction is an outpatient procedure that usually lasts from one to three hours, depending upon the amount of fat to be removed. If only a small amount of fat and a limited number of body sites are involved, liposuction can often be performed under local anesthesia, which numbs only the affected areas. A general anesthesia will be used for more extensive procedures or when it is performed in conjunction with other cosmetic procedures. Liposuction is performed by using a hand-held instrument (called a cannula) to vacuum the fat layer that lies deep beneath the skin. By pushing and pulling the cannula, the fat cells are broken up and then sucked out with a vacuum. While fat is removed by suctioning fat through small tunnels, the skin remains connected to the underlying muscles. This helps preserve the nerves and blood vessels supplying the skin and also helps to decrease the laxity of the skin. Dr. Wolfeld is also careful to leave a thin blanket of fat attached to the skin to prevent the cannula from getting too close to the skin and thus avoiding excessive rippling or other irregularities.
Tumescent Liposuction, a technique in which a medicated solution is injected into fatty areas before the fat is removed, is commonly used by Dr. Wolfeld for his liposuction patients. The fluid -- a mixture of intravenous salt solution, lidocaine (a local anesthetic) and epinephrine (a drug that contracts blood vessels) - facilitates fat removal, reduces blood loss and provides anesthesia during and after surgery. Fluid injection also helps to reduce the amount of bruising after surgery.
Ultrasound-Assisted Lipoplasty: This technique utilizes sound waves to break up fat deposits. A special cannula is used which is connected to an ultrasonic generator. As the is passed through the tissues, fat cells are ruptured, releasing their contents. The liquefied fatty emulsion is then removed. UAL improves the ease and effectiveness of liposuction in fibrous areas of the body, such as the upper back or the enlarged male breast. It is also commonly used in secondary procedures, when scar tissue is present and when enhanced precision is needed. In general, takes a little longer to perform than traditional liposuction.
The skin overlying the areas of liposuction may be numb or sensitive for several weeks. Bruising and swelling commonly occur as well. A compression garment is worn for 4-6 weeks to assist the healing process. Visible changes are immediately noticeable. Contour improvements delight patients and are generally permanent.
