Eyelid surgery, one of the most popular cosmetic procedures for both males and females, can have a very dramatic effect in restoring a more youthful, rested appearance. So often aging is first noticeable around the eyes. Crow’s feet appear, fine lines start showing at the outer corners, and the eyelids become droopy with fatty deposits and excess skin. You may have had a good eight hours sleep every night of your life, but will still be getting a tired look from these changes. When you look at your eyes in the mirror, you see sagging, puffiness, droopiness and wrinkles. You may wonder why you look tired and why people say you look tired, even when you don't feel it. When loose folds, sagging skin and bagginess are reduced, the eyes appear more naturally alert and brighter. For some people it even improves vision if the upper eyelid had been drooping so far as to interfere with vision. An eyelid lift can be performed on either the upper or lower lids or both. Dr. Wolfeld often performs a blepharoplasty (eyelid lift) in conjunction with other procedures, such as a facelift or brow lift. For example, if the upper eyelid condition is accompanied by sagging of the eyebrows, then a forehead or brow lift may be recommended. Smoothing of crow's feet may require chemical peeling or laser resurfacing procedures. Circles beneath the eyes caused by dark pigmentation may be treated with a bleaching solution or chemical peel. Patients with loose, lax facial skin may require a facelift at the same time.
One of several surgical techniques may be suggested to improve the appearance of your eyelids. The particular technique that Dr. Wolfeld recommends will depend on many factors such as the amount of excess fat and skin in the eyelid areas, the position of your eyebrows, and the condition of muscles around your eyelids. Dr. Wolfeld will spend an extensive amount of time in the consultation discussing these options with you and formulating an individual plan to have you feeling younger and better than ever.
Blepharoplasty is usually performed in an outpatient surgical facility under local anesthesia and mild sedation. General anesthesia may also be used. The procedure takes from 2-3 hours depending upon the extent of the condition and whether work is being done on both the upper and lower lids. For your safety during the operation, various monitors are used to check your heart rate, blood pressure, pulse and the amount of oxygen circulating in your blood. When surgery is completed, you will be taken into a recovery area where you will continue to be closely monitored. Your vision will be blurry as a result of ointment used to soothe and protect the eye during surgery as well as from the swelling that is a normal aftermath of eyelid procedures. There is surprisingly little discomfort, however, from the surgery. You will most likely be able to go home after a few hours.
For the upper eyelid, the incision is made in the natural crease of the upper lid. This incision allows Dr Wolfeld to remove or reposition fat deposits, tighten muscles and skin, and remove excess skin. Because the incision follows the natural contour of the upper eyelid, it will be well camouflaged when healed. For lower eyelid surgery, often an incision is hidden just below the lower lashes. Through this incision, excess skin, muscle and fat are removed, or fat may be redistributed to eliminate puffiness or bulges. Other adjustments to correct special problems such as muscle laxity may be performed. As in upper eyelid surgery, the incision is well camouflaged by natural creases. Often times, Dr. Wolfeld may decide that the best approach for removing excess fat is through an incision placed inside the lower eyelid. This technique requires no external incision, but it cannot be used to remove excess skin. A laser may sometimes be used in conjunction with this method to tighten the lower eyelid skin.
It is important to realize that the amount of time it takes for recovery varies greatly among individuals. The first evening after surgery, you should rest quietly with your head elevated. Dr. Wolfeld will instruct you to apply cold compresses to your eyelids. Although there will be some swelling and bruising which will fully diminish in a couple of weeks, many patients return to work within days of the surgery. The stitches will be removed (or dissolved) during the first week. Within the first week you will be permitted to use makeup, if desired, to conceal any discoloration. Scarring will be virtually invisible since the incisions are made in the natural lines and skin folds. As the swelling subsides, you will see a dramatic improvement in your eyes, their more alert and youthful appearance. Aesthetic eyelid surgery has the effect of making you look more rested, refreshed and vibrant.
